Moving a prerequisite down from Comprehensive changes scope and needs re-estimation before agreement. The 80% usefulness evaluation (at least 20 cases, DG-defined rubric, zero prohibited disclosures) is a separate Comprehensive gate; Balanced has no usefulness threshold.
